Platform: PhysioConnect
Electronic acceptance (checkbox / continued use after notice) constitutes signature.
1. Engagement
Provider is an independent professional, not an employee of Curanexa. Curanexa provides marketplace technology, verification workflows, booking tools, tracking, wallet/commission tooling, and related services.
2. Credentials and documents
- Provider warrants that license/PHCPT and identity documents submitted are true and current.
- Provider authorises Curanexa to store and review verification documents (CNIC, license, degree, etc.) for onboarding and compliance.
- Pending providers must not offer paid services until verified, where product rules require verification.
- Provider must notify Curanexa of licence expiry, suspension, or material change.
3. Clinical standards
Provider remains solely responsible for clinical care, informed consent for techniques, infection control, and professional ethics. Provider must keep accurate session notes as required by the platform (commission may depend on note submission and grace rules).
4. Fees and commission
- Agreed session fees must respect city floor pricing rules.
- Platform commission is always 20% of the agreed session fee for standard PT sessions (non-negotiable).
- Commission is recorded on the platform ledger according to product rules (including session-note gating).
- Wallet debt at or above the platform threshold may block new bookings until cleared.
- Payment proofs (e.g. JazzCash screenshots) may be required for finance verification.
5. Cancellations and strikes
Cancelling a confirmed booking results in an immediate strike. Three strikes in a rolling 30-day window may cause automatic suspension. Provider must follow Home-Visit Terms and patient privacy rules.
6. Location and tracking
Provider consents to use of location (including background location during “on my way”) for booking fulfilment and patient transparency, subject to device permissions.
7. Data and confidentiality
Provider must protect patient health information, use data only for care and platform purposes, and not export or misuse records. Private notes must not be disclosed improperly.
8. Suspension and termination
Curanexa may suspend or terminate access for false credentials, safety issues, fraud, harassment, or material breach. Provider may stop using the platform subject to settling outstanding platform obligations.
9. Liability
Provider indemnifies Curanexa against claims arising from Provider’s clinical acts, omissions, or misrepresentations, to the extent permitted by law.

Acceptance: By registering as a physiotherapist on PhysioConnect or tapping “I agree”, Provider accepts this Agreement.
